Mental Health Housing Support Services and Rapid Rehousing

This program assists people in making a smooth transition into living more independently in their community through transitional housing and support services.

Hours: Monday-Friday 8:00 am - 4:30 pm

Program Description & Details

This program assists people in making a smooth transition into living more independently in their community through transitional housing and support services.

Primary Service Activities

  • Biweekly meetings with Rise staff
  • Transitional housing living arrangement for up to 2 years based on eligibility
  • Independent living skills training such as budgeting, hygiene, shopping, meal planning, crisis resolution, and more
  • Housing support services such as landlord dispute resolution, advocacy, and skills to find long-term affordable housing
  • Rental subsidy programs, limited to $200 per month for 12 months, assistance creating a plan for sustainability after the subsidy ends. Limited financial supports for rapid rehousing.
