Rise

These programs are for those interested in finding a community job, but needing assistance finding opportunities that fit their goals and skills. Our Employment Consultants and Occupational Communication Specialists find jobs at local businesses and match people to opportunities based on their individualized plan.

Developmental/Intellectual Disabilities

Community Employment: Based on an individual's skills and interests, Rise employment consultants assist people 18 years and older develop a career plan and find competitive employment in the community. We use a strengths-based approach, taking people where they are at and assisting them in finding a job that is a good fit for each individual.  Rise also provides follow-up services after the person gets a job to ensure that they are successful on the job. This program is only for people who are funded by a waiver.

Deaf, Deafblind, Hard of Hearing

Minnesota Employment Center, Follow-Up Services: Rise's Minnesota Employment Center serves people who are Deaf, DeafBlind or Hard of Hearing through five Minnesota offices: St. Paul, Duluth, Faribault, Moorhead and St. Cloud. MEC provides career planning, job placement, occupational communication, and follow-up support services for these individuals as well as support for employers who currently have Deaf, DeafBlind or Hard of Hearing team members. Referrals come from Minnesota Vocational Rehabilitation, State Services for the Blind, Deaf and Hard of Hearing Services and self-referrals.

Mental Health

New Millennium/Employment Innovations: This program is for people with a serious and persistent mental illness (SPMI) who are unemployed or underemployed, and are residents of Chisago, Isanti, Kanabec, Mille Lacs, and Pine Counties. The purpose is to develop Individualized Placement and Support (IPS) plans consistent with individual interests, career goals, and plans through a mental health treatment team.

 

IPS Transformation Project: This program serves East Central Minnesota residents who have a diagnosed serious and persistent mental illness (SPMI) or serious mental illness (SMI) and are referred by the Adult Mental Health Initiative through Targeted Case Management and a Mental Health Treatment Team. All referrals must be generated through the Mental Health Treatment Team and its AMH Case Management partners. People participating in this program must also be co-enrolled with Minnesota Vocational Rehabilitation Services and be residents of Chisago, Isanti, Pine, Kanabec, or Mille Lacs Counties. It is intended for those who are currently unemployed or underemployed and looking to transition to alternate employment. We develop Individual Placement and Support consistent with the person's interests and career goals and plans through a Mental Health Treatment Team approach.

 

Pathways to Success: This program is for residents of Hennepin County who are living in housing supports at one of our identified providers. Referrals for this program must come from those housing support providers. Once eligibility is determined, we work with individuals to create a person-centered career plan including competitive and supported employment outcomes.

 

Employment Pays!: To enroll in this program, the person seeking services must be on Hennepin Health. This program is designed to develop competitive and supported employment outcomes consistent with the education, training, and person-centered career plans of individuals.

Pathways for Refugees: This program provides job placement and intensive support services for newly arrived refugees who are unemployed, live in either Hennepin or Ramsey Counties, and are recipients of Refugee Cash Assistance, Diversionary Work Program, Non-Public Assistance, or MFIP support. Referrals come from the Minnesota Department of Human Services.

Minnesota Families Investment Program (MFIP): This program provides job placement and support services for MFIP recipients who are unemployed and having a difficult time re-entering the job market. Rise employment consultants use a person-centered approach to create a plan that matches the individual's own career goals.
