Mental Health and Housing Support Services in East Central Minnesota (Region 7E: Chisago, Isanti, Kanabec, Mille Lacs, and Pine Counties)

At Rise, we believe that everyone deserves a safe and affordable place to live. Rise can assist you in making a smooth transition into living more independently in the community.

We offer a wide array of mental health support, housing assistance, outreach and referral, and advocacy services in East Central Minnesota to individuals, families, and transition-age youth whose lives are affected by mental illness. We work with people on mental health case management, identifying other community resources, and applying for public assistance and/or rental subsidies.

We also assist households in reaching their personal level of self-sufficiency and mental health recovery.

Our programs can assist you in tackling many housing challenges, including:

  • Locating safe and affordable housing including emergency shelter
  • Independent living skills
  • Household management skills such as budgeting and housekeeping
  • Landlord/tenant rights and responsibilities
  • Mental health recovery, and
  • Crisis assistance.

We can also assist you with:

  • Addressing housing and credit barriers
  • Negotiating with your landlord
  • Identifying community resources, and
  • Applying for public assistance and/or rental subsidies.

Transitional housing and rental subsidies

Rise’s Transitional Housing Program (THP) is time-limited and targeted to households who are homeless or for whom market-rate housing is not available due to poor rental, credit and criminal backgrounds.

Rental subsidies can assist in paying a portion of your rent for one year, up to $200 a month, if you live in Greater Minnesota.

Both our THP and rental subsidies require recipients to meet with Rise staff twice a month and participate in a day commitment consisting of work, school, volunteering, or mental health treatment at least 20 hours per week.

Employment assistance through Rise programs

Rise also offers assistance in qualifying for and maintaining employment. Rise employment team members provides planning services and supports so you can find and maintain a job which meets your individual needs and skills, and provides the support needed to be successful in the career field of your choice.

We can also assist you with benefits planning to help you determine how best to maintain the medical and income assistance benefits you need and are entitled to while earning a paycheck.
